CARE AT HOME
Remaining in the comfort of our own home would be most people’s wish, and with Key Care & Support by your side, this is possible through our dedicated ‘Care at Home’ team.
Key Care & Support has a range of services that we can individualise to meet your requirements.
This service is designed to ensure you can live as independently as possible, assisting daily routines that may be becoming difficult to fulfil on your own. This could be such things as assisting with meal preparation, dressing, bathing or reminding to take medication. There are many other areas that we can assist with to ensure you’re supported throughout your daily routine; we can supply Qualified Nurses, experienced Clinical Support Workers and Home Carers that have undergone training to meet your specific requirements, including bespoke training put into place following your initial assessment. A personalised plan will be completed with you and regularly reviewed with you by our experienced Care at Home team, ensuring you are always involved and happy with the support we provide.
Our Palliative Care Service is designed to support you or a relative that has been diagnosed with a terminal illness, ensuring that you can remain in the comfortable surroundings of your home with your family and loved ones. A personalised plan will be agreed with your choices and preferences. Your request for dignity and respect are central to our planning. We want you to have the quality of life that you and your family deserve.
Hospital Discharge Support
This service allows you to recuperate in the comfort of your own home, without having to transfer to a Care Home or stay in hospital any longer than necessary. We can assist with daily activities during your recuperation or provide a full team of Nurses and Clinical Support Workers for 24 hour Nursing Care based on your requirements. We will work with you and the hospital team throughout the process to ensure a smooth transition. We can provide a flexible service, such as short visits to assist with personal care, travel to hospital/Doctor’s Appointments, giving family/friends a break and support throughout the day and night.
Live in Support
This is a personal, one-to-one service and therefore a staff member will be matched to your personal requirements in consultation with you. The Live in Support Service allows you peace of mind and provides wellbeing by enabling you to remain in your home with care continuity. Your chosen carer will move in to your home on a 2 to 3 week rota, providing ongoing support and care.
The Live in Support service provides all the requirements to assist in this area. It is also a service that can provide companionship throughout the day and the comfort that someone you can trust is close at hand during the night. The service can be on a short or long term basis, with only a very small team required and so providing you with continuity.
