We provide bespoke, complex care in the home to clients with a range of complex needs, throughout England & Wales. Enabling independence, choice, and the chance to live life to the full. For some people, receiving care in their own homes ensures greater independence and a greatly improved quality of life. We are registered with the CQC in England to provide complex care in the community. Our professional healthcare staff have the expertise to help people with a range of complex health and social care needs in their own home. Our staff receive ongoing training, support and supervision to ensure care is personalised to each person's individual needs.
Our Palliative Care Service is designed to support you or a relative that has been diagnosed with a terminal illness, ensuring that you can remain in the comfortable surroundings of your home with your family and loved ones. A personalised plan will be agreed with your choices and preferences. Your request for dignity and respect are central to our planning. We want you to have the quality of life that you and your family deserve.
Hospital Discharge Support
This service allows you to recuperate in the comfort of your own home, without having to transfer to a Care Home or stay in hospital any longer than necessary. We can assist with daily activities during your recuperation or provide a full team of Nurses and Clinical Support Workers for 24 hour Nursing Care based on your requirements. We will work with you and the hospital team throughout the process to ensure a smooth transition. We can provide a flexible service, such as short visits to assist with personal care, travel to hospital/Doctor’s Appointments, giving family/friends a break and support throughout the day and night.
Live in Support
This is a personal, one-to-one service and therefore a staff member will be matched to your personal requirements in consultation with you. The Live in Support Service allows you peace of mind and provides wellbeing by enabling you to remain in your home with care continuity. Your chosen carer will move in to your home on a 2 to 3 week rota, providing ongoing support and care. The Live in Support service provides all the requirements to assist in this area. It is also a service that can provide companionship throughout the day and the comfort that someone you can trust is close at hand during the night. The service can be on a short or long term basis, with only a very small team required and so providing you with continuity.
Adopting a fresh and modern approach, we understand that when using an outside agency to support staffing requirements within childcare services, there is no margin for error in the quality, vetting and practices of the provider and worker alike. Embracing the government led ‘every child matters’ agenda and the aims promoted within it, Key Care & Support ensure that each and every requirement is met with a passion and vigour that is unmatched by our competitors. Key Care & Support have extensive knowledge of supporting residential homes for children and young people with a variety of complex needs including, for example, social and emotional difficulties, challenging behaviour, profound autism and learning disabilities, physical disabilities, ADHD and Asperger’s.
We provide caring, supportive, experienced and appropriately trained staff to social care settings, including Individual Personalised Support, Supported Living and all other types of services within the local community. Key Care & Support believe that individuals who require support have individual requirements and preferences should always have a choice and control over how they live their lives. This is our philosophy and one which is instilled within our staffing resource and throughout our business. Committed to principles of individualisation and personalisation, Key Care & Support will ensure the right level of support is implemented and constantly reviewed. Everyone is allocated a Support Manager, who will work with them to assess their requirements and ensure we provide a service that meets their specific requirements. At Key Care and Support, we believe that ‘Quality of Life Counts’.
